Next Level Primary Care offers two options to enroll: online and in-person. Enrolling online takes less than 2 minutes, because we only need basic contact and billing information:
- Name, Address, Email, Phone Number, Date of Birth, Gender
- Add your payment info and you’re done!
That’s it! Once you’ve enrolled, a member of our staff will reach out to you to answer any questions you may have and to schedule your first appointment to establish care.

To enroll in-person, simply schedule an Enrollment Appointment at the clinic by using the form below. When you come in, we will need to collect the following basic information:
